Unfortunately I don’t know of any production systems using the separation of data and logic. The resources I have listed are the best I can find.

The first link in the Proxy resources is to an ERC with links to the ZepplinOS and AaragonOS githubs which have examples of them using the proxy contract which they plan to use in production at some point.
